Section 20-700 APPLICATION FOR SITE PLAN REVIEW Section 20-701 The following materials when submitted in their entirety to the Planning Commission, shall constitute an application for a Site Plan Review. Section 20-702 Eight copies of a completed application containing
the following information: 2. Proof of property ownership, and whether there are any options on the property, or any liens against it. 3. A signed statement that the applicant is the owner of the property or officially acting on the owner's behalf. 4. The name and address of the owner(s) of record if the applicant
is not the owner of record (or firm or corporation having a legal
or equitable interest in the land) and the signature of the owner(s). 6. Name and address of the developer (if different from the applicant). 7. Name and address of all registered professionals involved with the proposed project including the engineers, architects and surveyors. Plans shall contain the seal of a minimum of one of the registered professionals responsible for the preparation of the same. 8. Project description, including the total number of structures, units, bedrooms, offices, total floor area, parking spaces, carports or garages. projections of employment levels for commercial and/or industrial uses. amount of recreation and open space, type of recreation facilities to be provided and related information as pertinent or otherwise required by this Ordinance. 9. The gross and net acreage (exclusive of road and utility easements) of parcel or parcels included in the project. 10. Project beginning and completion schedule (by phase if the entire project is to be divided into phases). 11. Written narrative stating anticipated impacts on existing infrastructure (including traffic capacity of streets, schools and existing utilities), community facilities, and on the natural environment of the site and adjoining lands. Narrative shall contain information on the potential for water and air pollution; any noise or glare that may be created, any potential fire or safety hazards; on emission of dangerous or obnoxious matter; and on the proposed treatment of any such conditions to maintain the same within the confines of the property and within the permitted limitations of this Ordinance. The narrative shall also contain information on how all sewage and wastes will be disposed. 12. A listing of types and quantities of hazardous substances and polluting materials which will be used, stored or generated on-site at the facility (including fuels and cleaning solvents). 13. The proposed Master Deed if the development is to take place under the auspices of the Condominium Act. Section 20-703 Eight full size site plan maps and one 11 " x
17" shall be submitted as a component of the application package.
The site plan maps shall consist of accurate drawings at a scale
of 1 "=100' or less, showing the site and all land within
150' of the site. If multiple sheets are used, each shall be labeled
and the preparer identified. Each set of site plan maps shall depict
the following: 3. The applicant shall provide written evidence that the proposed development of the property will conform to the provisions of such Soil Erosion and Sedimentation Control as may be in effect in accordance with the Antrim County Soil Erosion, Sedimentation Control and Storm Water Ordinance, Public Act 347 of 1972 has been replaced with Public Act 451 of 1994. as amended. 4. The applicant shall provide written evidence that a sewage treatment or disposal system has been approved by the County Health officer and Waste Water Division of the Department of Environmental Quality when applicable and that a sewage treatment or disposal system is in conformance with this Ordinance, including but not limited to setbacks from water bodies, height above water level and other applicable requirements. 5. The applicant shall provide evidence that the cutting and removing of trees and other native vegetation will be performed according to the following standards: a. Cutting shall be done in a manner which prevents erosion, preserves rare species of trees and other vegetation, preserves scenic qualities and provides screening from neighboring properties and adjoining roadways. b. Clean cutting shall be permitted when necessary to accommodate building sites as follows: Grading or clearing of a site shall not commence prior to issuance of an Environmentally Sensitive Land Use Permit as required by this Ordinance, a building permit and other applicable permits. For single family dwellings and amenities for the private (noncommercial) use of the occupants of the single family dwelling, said clean cutting shall not exceed a ratio of 10,000 square feet pen acre. 6. The applicant shall maintain a thirty foot (3 0') Shoreline Protection Strip which is measured from the mean high water line. 7. Designation of all areas to be used for the outdoor storage,
display or sale of 8. Location and elevations of existing water courses and water
bodies, including county drains and man-made surface drainage ways.
flood plains and wetlands, both regulated and unregulated. 10. Proposed location of accessory structures, buildings and uses. including but not limited to all satellite dishes, antennas, flagpoles, light poles. bulkheads, docks. storage sheds. transformers, air conditioners, generators and similar equipment. and the method of screening, where applicable. 11. Location of all existing public and private easements including road and utility right-of-ways on or abutting the property. 12. Location of and dimensions of proposed streets, drives, curb cuts, and access easements, as well as acceleration, deceleration and passing lanes serving the development. 13. Location, design. and dimensions of existing and/or proposed curbing, barrier free access, carports, parking areas (including type of material. proposed to be used for surfacing), fire lanes and all lighting thereof. 14. Location, size and characteristics of all loading and unloading areas. 15. Location and design of sidewalks, walkways, bicycle paths and areas for public use. 16. Location of water supply lines and/or wells, including shutoff valves, and the location, size and design of drainage pipes and tiles, storm sewers, retention or detention ponds, waste water lines, clean out locations, connection points and treatment systems, including septic systems, if applicable. 17. Location of all other utilities on the site including but not limited to natural gas, electric, cable TV, and telephone. 18. Proposed location, dimensions and details of common open spaces and common facilities such as community buildings or swimming pools if applicable. 19. Location, size and specifications of all signs. 20. Exterior lighting locations with area of illumination illustrated
as well as the 21. Location and type of fencing, walls, and other screening devices. 22. Location and specifications for all proposed perimeter and internal landscaping and other buffering features. For each new landscape material, the type and size at the time of planting must be indicated. All vegetation to be retained on the site must also be indicated, as well as its general size and specific location. 23. Location, size and specifications for screening of all trash receptacles and other solid waste disposal facilities. 24. Identification of existing site amenities and/or unique natural features. This includes historical, cultural and environmental features including: rock outcrops. steep slopes, major tree stands, historic structures, etc. 25. Identification of any scenic views onto or from the site and to or from adjoining properties. 26. North arrow, scale and date of original submittal and last revision. 27. For facilities or uses where hazardous substances as defined by the Resource Conservation and Recovery Act are stored, used or generated, the following information shall be provided: a. Location and size of interior and exterior areas and structures to be used for storage, use, loading/unloading, recycling, or disposal of hazardous substances. b. Location of all underground and above ground storage tanks for such uses as fuel storage, waste oil holding tanks, chemical storage, hazardous waste storage, collection of contaminated storm water or wash water, and all similar uses. c. Location of exterior drains, dry wells, catch basins, retention/detention areas and other facilities designed to collect, store or transport storm Water. The point of discharge for all drains. d. Delineation of areas on the site which are known or suspected to be contaminated, together with a report on the status of site cleanup. e. Copies of all Federal/State/County permits required. 28. Location of fire hydrants, underground water storage tanks
or other sources of 29. Site plans shall take into consideration the location and extent of any contaminated soils and/or groundwater on the site, and the need to protect public health and the environment.
a. Existing topographic elevations at two foot (2') intervals, proposed grades and direction of drainage flows. b. The location and type of existing soils on the site and any certifications of borings. c. Location and type of significant existing vegetation to be retained as well as those proposed to be removed. d. Elevation drawings of buildings, signage and other structures. f. Flagging on the site to demonstrate location size and height of proposed structure or other proposed site features. Section 20-704 Upon the request of the applicant, the Planning
Commission may waive certain requirements of the site plan submittal
requirements listed above, provided the Planning Commission shall
document in the minutes of a public meeting specific Section 20-705 All applications for Site Plan Review as set forth
in this Ordinance shall be accompanied by a filing fee specified
in the Forest Home Township Fee Schedule. Additional funds may
be required of the property owner by the township should outside
professional assistance be required in the review of the application.
Such additional fees for required outside professional assistance
shall not exceed actual costs to the township, nor shall they be
in excess of charges standard in the industry or profession. |
